Our current rates (effective January 2026) are $160 for an evaluation and $140 for follow-up sessions. After each session, a superbill will be available for you. You can submit this superbill to your insurance provider if you choose—many plans offer out-of-network benefits, and a large percentage of our clients are able to recoup part of their investment. We also accept HSA/FSA cards.

Have a high deductible? In many insurance-based PT settings, you’ll still pay high out-of-pocket costs until your deductible is met. Out-of-network care—like what we offer at Motion Evolved—is often more cost-effective, more efficient, and more individualized.

Special Arrangement With Medicare & Tricare (Select)

Did you know Medicare beneficiaries are often restricted from accessing premium out-of-network physical therapy? At Motion Evolved, we’ve chosen a non-traditional provider status with Medicare and Tricare Select so that you can still access high-quality, 1:1 PT without being limited to in-network clinics.

You will pay for your care at the time of service, and Motion Evolved will submit your claim for you. You can expect to receive approximately 70% reimbursement, mailed directly to you by your insurance carrier. Processing of claims typically takes 4-6 weeks.

Pricing for Medicare and Tricare differs slightly from standard out-of-network rates and is based on the services provided during your visit:

• Initial Evaluation: $175–$220 (anticipated reimbursement $120-$153)

• Follow-Up Sessions: $123–$137 (anticipated reimbursement $83-$95)
